Online ApplicationThe preferred method to complete the application for the primary certification exam is by using the online application. The application cycle for the 2008 exam is open from March 1 to June 1. A sample application form can be viewed onscreen or downloaded by clicking Sample Application. Application forms may also be obtained by sending an email request or writing to the American Board of Preventive Medicine office in Chicago, Illinois. We strongly encourage all applicants to use the online application process. There is an additional $200 fee for processing paper applications for the examinations in Aerospace Medicine, Occupational Medicine, or Public Health & General Preventive Medicine. If you need help deciding which pathway is the correct one for you to use, please refer to the pathway descriptions under the Certification Requirements page. The applications for the Undersea & Hyperbaric Medicine subspecialty and the Medical Toxicology subspecialty examinations are not available for completion using the online process.
